This morning I encountered one of the UM Health System's online questionnairres that has the instruction not to pick No for any of the questions, but then won't let you submit it unless you answer all of the questions. I actually called them up and asked about it. They said that the instruction at the top (where it says not to pick No for anything) is "a little glitch," and that we should ignore it.
