Oh, good news on those forms-- The big one says at the top that if I'm already a UofM health system patient, I don't have to fill it out. I am, very definitely, already a patient in the system. That's a relief. It's always a PITA to list all of my medications (and dosage) and all of my allergies and all of pre-existing conditions and so on.
The shorter form, I may or may not bother to fill out. It's a release to let the medical folks give verbal information about my health to the friends and family that I specify. I suppose it might be useful to have on file some day, but I don't feel any urgency since I'll be going to this appointment alone. I'm also not sure who, besides Scott, I'd put on it. I'm not convinced Cordelia is old enough (I'm not sure she isn't, however). My parents live so very far away that they're unlikely ever to be around. Scott's sister? Scott's parents? My brother who lives two hours away? There are five spaces on the form, so I could, in theory, add all but one of them. (I'd probably skip Scott's father. He's not likely to be there without Scott's mother, but she might well be there without him.)
I've got a couple of hours to think about it.
They suggest that I arrive half an hour before my appointment, but that seems to be predicated on the assumption that I'll need time to park. Since I'll be going by cab, I think arriving fifteen minutes ahead should be fine.
There doesn't appear to be anything I'm supposed to do or not do before the appointment. That's good to know. I was wondering if, as with a mammogram, I was supposed to skip the deodorant/anti-perspirant.
