The Cancer Center actually plans to mail me a paper health history to fill out and bring to my appointment. Nobody in the University Health System has done that in years. Everybody else has the forms online, accessed through the patient's web portal account, unless the patient is brand new to the system and so doesn't yet have an account.
They're sending it UPS, too, but assured me that I won't have to sign for it.
But filling out a form online would be a heck of a lot easier.
